PrimeCareHealth (PCH) is a nationally recognized health care network providing comprehensive family medical care to people of all ages, regardless of patients’ ability to pay. As a member of Investing In Communities®, PCH can receive funding as a result of any real estate transaction – home sales, home purchases, or commercial leases. Already, $30,000 has been raised through a single office lease transaction. As more supporters use this platform before engaging an agent, PCH will have more funds to provide excellent community-based medical care.

Service Areas and Locations

PCH was launched in 1992 as a 501(c)3 nonprofit to ensure that the underserved and uninsured residents on Chicago’s West and Northwest sides would not have to forego quality, affordable health care. PCH attained full status as a federally qualified health center (FQHC) in 2000. The PCH network includes the following locations and facilities:

Dental Care, Counseling, and Prenatal Care

Staffed by caring physicians, general practitioners and nurses, the essence of PCH is a community focus on primary medicine. Since the passage of the Affordable Care Act in 2010, PCH has expanded the range of health services it provides to the adults, seniors, women, and children in the communities it serves. Behavioral health counseling, prenatal health care, dental, and individual and group therapy are now among the wide range of services designed to meet the medical needs of individuals of all ages.

Emphasis on Prevention

PCH places a special emphasis on providing preventive and primary care services. It also supports its practice with community outreach and routine health services such as cancer screenings, immunizations, weight management and dietitian services. PCH’s focus is on open and candid communication and goal-setting helps patients develop the skills they need to understand, manage, and successfully take charge of their own health care.

As a tax exempt 501(c)3 organization, PrimeCareHealth is eligible to receive at least 70% of the referral fee when you use Investing In Communities (IIC) to sign up with ANY real estate broker to handle your residential or commercial property transaction. Referral fees are a common expense paid by brokers only to other brokers for much desired referrals. As a licensed broker with a charitable mission, IIC is able to accept referral fees and pass most of those funds to your charity of choice - at no cost to you.
